INSTRUCTIONS FOR OBTAINING CERTIFICATES OF INSURANCE ON-LINE

Please follow these instructions for your Certificates

1. Sign is using this form:.

   In order to use eCertsONLINE you must have been assigned a User ID and Password. Input the User ID and Password here then click on Login. eCertsONLINE Login     User ID (Your e-Mail Address)   Password

1. Enter your USER ID (your own e-mail address)
2. Enter your PASSWORD (this is the first five (5) characters to your company name
3. Press LOGIN
2. Click on Issue a Certificate of Insurance button  in the My Forms field.
3. Select the type of Certificate you need.
4. Select ADD INTERESTS.
5. Complete the INTEREST INFORMATION section.
6. Under DELIVERY OPTIONS select “send form via e-mail.”
7. Type-in your e-mail address.
8. Click on SAVE
9. Click on ISSUE
10. Click on ISSUE FORMS
11. Select CONTINUE
12. If you need more Certificates click on RETURN TO STEP TWO.
13. If you do not need any more Certificates click on LOGOUT.
14. Your Certificates will be sent by e-mail.
