INSTRUCTIONS FOR OBTAINING CERTIFICATES OF INSURANCE ON-LINE
Please follow these instructions for your Certificates
- Sign is using this form:.
- Enter your USER ID (your own e-mail address)
- Enter your PASSWORD (this is the first five (5) characters to your company name
- Press LOGIN
- Click on Issue a Certificate of Insurance button in the My Forms field.
- Select the type of Certificate you need.
- Select ADD INTERESTS.
- Complete the INTEREST INFORMATION section.
- Under DELIVERY OPTIONS select “send form via e-mail.”
- Type-in your e-mail address.
- Click on SAVE
- Click on ISSUE
- Click on ISSUE FORMS
- Select CONTINUE
- If you need more Certificates click on RETURN TO STEP TWO.
- If you do not need any more Certificates click on LOGOUT.
- Your Certificates will be sent by e-mail.
